Address Change Request

InTouch Credit Union is required by the Customer Identification Program and USA Patriot Act to have a physical street address on file for you. You must fill out Section 1 with your physical address, even if you receive all your mail at your P.O. Box. Please complete Section 2 if you want your mail sent to a P.O. Box or temporary address.
